Step 1:Complete the claim form
Step 2: Submit claim form together with the Specialist Report (to be completed by your attending specialist) and other supporting documents i.e. laboratory test results, hospital invoices and receipts etc.
Step 3: Email the completed claim form & Specialist Report to 'Claims@chinalife.com.sg'

Under the Full SMS Sender ID Registry (“SSIR”) Regime, all organisations intending to use Sender IDs in their SMS messages to Singapore mobile users must first register the Sender IDs via the SSIR portal. All non-registered Sender IDs will be marked as “Likely-SCAM”.
China Life Insurance (Singapore) has registered ChinaLifeSG as its Sender ID and customers receiving SMS messages from this Sender ID can be sure that they are sent from us.
